When i use this i get the wrong city returned. Is there some sort of database that i need to install on my server? I am in Chino, California but it tells me I am in "Pouter, Pennsylvania". I noticed that other sites i visit seems to know that i am in Chino.

The information comes from NetGeo service. In some cases it will return inaccurate information because in the Internet the owners of each IP range change. If the new owners are located in new places but they do not update the records of information that are checked by Netgeo, Netgeo will return outdated information.

I just experienced this issue and was surprised to find out that NetGeo is no longer being maintained - I think a new service will have to be used and appropriate changes/new class will have to be written!

Right. There are many other services similar to this. The problem is that most of them constrain the number of queries you can make for free, if any at all.

I don't thing there is an free database for IP to location information. The problem is that such databases need to be updated as the net IP ranges switch places and that takes a look of work to be given away for free.
